Truck access and site risks

Walk the entire route from the maintained public road to the setup or unloading point before checking these items. Customer directions never override the driver’s safety judgment.

Route, room, and trafficI have inspected, or arranged for someone familiar with the site to inspect, the complete truck route. I will provide adequate width, height, turning and backing room, grade, gates, and traffic control for a fully loaded truck, and I will keep people, pets, vehicles, equipment, and stored materials clear of the travel and unloading area.
Buried and concealed hazardsI will identify, mark, and disclose known or suspected hazards before delivery, including underground water, sewer, gas, electric, communication, and irrigation lines; septic tanks and drain fields; wells, culverts, vaults, old trenches, recent excavation, and uncompacted fill; and holes, stumps, rocks, debris, drop-offs, or other hazards concealed by grass, weeds, snow, mud, standing water, or stored material. I understand public utility marks may not identify private or customer-owned facilities.
Overhead and side clearanceI will identify and disclose low branches, overhead power or communication lines, wires, rooflines, eaves, gates, fences, signs, scaffolding, and other overhead or side obstructions. I understand overhead power lines must be treated as energized. I will not direct a truck beneath, beside, or near an obstruction unless the driver determines there is adequate clearance for the vehicle and any operating equipment.
Ground support and changing conditionsI understand that ground supporting a pickup or passenger vehicle may not support a fully loaded concrete or material truck. I have considered soft, wet, thawing, recently excavated, filled, undermined, or uncompacted soil; lawns, shoulders, slopes, ditches, pavement edges, driveways, culverts, bridges, and slabs; and weather or site changes that could weaken the route, setup area, or unloading point. I will notify dispatch if conditions change.
Mandatory customer-supplied ground guide and driver stop authorityTreasure State Concrete, Inc. policy requires me to provide one designated adult ground guide identified on this form before a company truck leaves a maintained public road or enters, backs, maneuvers, unloads, spreads, or exits the jobsite delivery area. Before movement begins, the ground guide and driver will confirm the planned route, known hazards, worker-free area, communication method, and agreed hand signals. The ground guide will wear high-visibility apparel; remain continuously visible to the driver from a safe position outside the vehicle's path, blind areas, and pinch points; watch for people, vehicles, structures, utilities, overhead or concealed hazards, unstable ground, and property that could be damaged; and perform no unrelated task or use a phone, headphones, or other distraction while guiding. Only the designated ground guide may give routine movement directions, although anyone may signal an emergency stop. The driver retains control of the truck, may reject any direction, and will stop immediately if contact with the ground guide is lost, communication is unclear or conflicting, or conditions appear unsafe. Movement will not resume until safe communication and conditions are reestablished. If a compliant ground guide or safe route is not available, the driver may remain at a safe staging point, stop, delay, relocate, or refuse the requested movement or delivery.
Recovery, towing, and property damageI understand a truck stuck because of soft or unstable ground may require qualified heavy recovery and result in a towing bill. To the extent caused by my failure to perform the site and ground-guide duties in this acknowledgment, inaccurate or incomplete site information, an undisclosed customer-controlled hazard, or a direction given by me or my ground guide, I am responsible under applicable law and the controlling written terms for resulting delay, recovery, towing, returned-material charges, or property damage. I will not attempt to pull or recover a stuck truck with site equipment unless Treasure State Concrete, Inc. authorizes the recovery plan. Treasure State Concrete, Inc. remains responsible to the extent a loss is caused by its own negligence or misconduct. Nothing in this acknowledgment waives any right or responsibility that may not lawfully be waived.

Confirmed-order and material conditions

Review the delivery responsibilities and material limitations that apply only after an order has been confirmed separately with dispatch.

Final order verification before production or deliveryBefore pickup, loading, or delivery—and, for ready-mix, before batching begins—I will verify the product or mix, intended use and specification, quantity, additives or admixtures, color, delivery or pickup address, unloading or placement method, schedule, and all special instructions. I will report corrections before production or loading begins. Phone quantities and pricing may be estimates until confirmed on the applicable written quote, order, or delivery ticket.
Delivery responsibility and chargesI accept responsibility for ordered material that is delivered or placed as I direct, subject to the applicable written quote, delivery ticket, sales terms, and rights that may not lawfully be waived. I understand that delays, rejected access, standby time, returned material, cleanup, recovery, or towing may result in additional charges when allowed by those terms.
Ready-mix batching, hydration, and timeI understand ready-mix concrete cannot be unbatched. When cement, water, and the other materials are combined during batching and mixing, hydration—the chemical reaction that leads to stiffening, setting, and hardening—begins. Normal waiting at the plant, in transit, at the jobsite, or in the truck does not pause or reset that process. Temperature, mixture proportions, approved admixtures, travel, and handling can change the rate, so no fixed working time is promised. I will have the site, access, crew, forms, equipment, and placement plan ready before the truck arrives and will communicate delays immediately. Once Treasure State Concrete, Inc. begins batching concrete to the verified order, production is committed and the customer is responsible for the order and applicable charges under the controlling written terms, even if the site, crew, access, schedule, or requested quantity later changes. This does not require the customer to accept nonconforming material or waive any right that may not lawfully be waived.
Concrete washdown and containmentIf my order includes ready-mix delivery or concrete pumping, I will provide a designated washdown area on private property that the property owner has approved, or I will arrange a temporary washout tub or other containment option with Treasure State Concrete, Inc. before delivery. I understand washdown is not permitted on streets, shoulders, sidewalks, gutters, rights-of-way, storm drains, ditches, waterways, or any other public property. Availability, placement, capacity, service, and disposal responsibilities for a temporary tub must be confirmed before delivery.
Concrete cracking and shrinkageI understand concrete naturally shrinks and may crack. Treasure State Concrete, Inc. does not guarantee a crack-free result, and control joints reduce but do not eliminate cracking.
Placement pace, strike-off, finishing, and appearanceI understand the receiving crew must be ready before discharge begins. For slabs and other flatwork, concrete should be deposited as near as practical to its final position and the crew should progressively place, consolidate as required, strike off or screed, and initially level manageable sections as placement advances. The crew should not assume that the entire placement can be discharged first and struck off afterward. Later floating and finishing must still be timed to the concrete’s actual bleeding and surface condition and should not be performed over visible bleed water. The appropriate sequence and method depend on the approved plans and specifications, mixture, placement method, weather, and qualified field judgment. Final strength, surface quality, color, and appearance also depend on correct placement, consolidation, finishing, jointing, curing, timing, protection, and site conditions.
Ready-mix supply scopeI understand ready-mix supplied to my requested specification does not include placing, screeding, finishing, curing, saw cutting, testing, or protecting the concrete unless Treasure State Concrete, Inc. separately agrees to that work in writing.
Material characteristics and truck spreadingI understand natural aggregates may vary in color, gradation, shape, and moisture. When Treasure State Concrete, Inc. agrees to truck-spread any delivered material, spreading is approximate. Exact depth, width, location, grade, quantity distribution, compaction, or finished surface is not guaranteed. The driver may stop or decline spreading when access or conditions are unsafe.
Inspection and reportingI will inspect the material and delivery promptly and report a suspected shortage, wrong product, visible defect, or delivery concern to dispatch as soon as reasonably possible and before using the material when practical.
